After a Hospital Stay in Eastleigh

Most older adults in Eastleigh who need hospital care are admitted to Southampton General Hospital (University Hospital Southampton) — the region's largest acute site, with its stroke and neurosciences services in the Wessex Neurological Centre. Some residents in the north of the borough, towards Chandler's Ford and Fair Oak, may instead be treated at the Royal Hampshire County Hospital in Winchester, depending on where they were admitted.

Once you are discharged, your NHS rehabilitation at home usually continues through the community teams of Hampshire and Isle of Wight Healthcare NHS Foundation Trust — such as the community stroke team or an integrated care team — alongside Hampshire County Council's reablement service. That community rehabilitation and reablement is typically time-limited, usually to around six weeks (NICE NG74). It is worth checking your discharge paperwork for the name of the team taking over.

When that NHS support comes to an end, we can continue the work at home — unhurried, specialist physiotherapy and occupational therapy for as long as it is helping.
